Whether it’s a routine draw for blood testing or the insertion of an IV line for a fluid supply, a 20 gauge needle is often an appropriate choice for the job. Although it is slightly larger in outer diameter than the most commonly used needle gauges, the 20g is ideal for effective injection or withdrawal of fluids.
When Do You Use a 20g Needle?
A 20 gauge needle is strong enough to administer subcutaneous and intramuscular injections alike. Along with 16 and 18 gauge needles, the 20g is also the size of choice for epidural injections to block pain. Another use for the 20 gauge is extraction of tissue for biopsy.

What Factors Determine the Needle Sizes?
Outer and inner diameter plus wall thickness are the criteria that determine needle gauge. Each size is standardized according to the Birmingham scale. Those with larger diameters have a larger gauge than those with smaller diameters.
A 20g needle has the following dimensions with the allowable variations shown in parentheses:
- Outer diameter: 0.03575 in. (variation 0.00025 in.) or 0.9081 mm (variation 0.0064 mm)
- Inner diameter: 0.02375 in. (variation 0.00075 in.) or 0.603 mm (variation 0.019 mm)
- Wall thickness: 0.00600 in. (variation 0.00025 in.) or 0.1524 mm (variation 0.0064 mm)
- Average length: 0.5 to 1.5
To make is easier to identify a needle’s gauge on sight, the ISO6009 color-coding system applies to hypodermic needles too. The ISO6009 system assigns a specific color to each gauge of single-use needle. The Luer lock or taper, a plastic collar that connects the needle to the syringe, bears the ISO6099 color. 20 gauge needles have yellow Luer locks.
