What Is a Trocar?

A trocar, which may also be written "trochar", is a medical device which is used during laparoscopy procedures. A laparoscopy is a minimally invasive operation performed in the abdomen or pelvis that involves the use of only small incisions and a miniature camera to guide the surgeon as they operate.

Trocars are a key part of the laparoscopy procedure, as they are placed inside the abdomen during the surgery. Once in place, the trocar effectively serves as a gateway or portal to facilitate the placement of other instruments like scissors and staplers, while also allowing fluids and gasses to escape from the body.

Trocars have been used for several centuries. They were originally used to relieve a build-up of gasses or fluids in the body, and they consist of an awl (a metal or plastic tip), a cannula or tube, and a seal at the top.

How Does a Trocar Work?

Trocars are used to make puncturelike incisions in the outer tissue of the abdomen during a laparoscopy procedure. These small incisions allow surgeons to insert other surgical instruments into the abdomen without the need for large incisions or a more invasive procedure. For the best results, the patient will first be placed in the Trendelenburg or reverse Trendelenburg position and a small incision will be made in the abdomen. The trocar can then be pushed in at a perpendicular angle to the incision, directly into the abdomen.

Trocar Sizes 

There are multiple sizes of trocars, with some being significantly larger than others. The three most common sizes are 5mm, 10mm, and 12mm in length.

Trocar Types

We can divide trocars into a range of types, based on their design and usage. A lot of trocars in use in hospitals nowadays are classed as disposable trocars, which means that they are disposed of after a single use, while others may be sterilized and reused; these are known as reposable trocars or reusable trocars. We can also divide trocars into different types based on their shape. In the past, it was very common to find three-point trocars due to their three-sided design. However, three-point trocars are less commonly-used in modern times, gradually replaced by alternatives like blade-free trocars or dilating-tip trocars, which offer additional safety for the patient and reduce the risks of accidental organ or blood vessel punctures.

The main use of trocars is to aid in the insertion of various surgical tools and implements during a laparoscopy procedure. They also help to release gasses and fluids that have built up within the abdomen.
